This is a Five/Six Way cruciform (because they are shaped like a cross) medal. The Four/Five/Six Way refers to the number of saints invoked. On this one, at the top is the (1) Sacred Heart of Jesus ('Behold the Heart that has loved Man so well, and been loved so little in return.'), (2) The Holy Spirit in the form of a dove often represents the presence of God's Grace upon us, but is also the source of divine inspiration, the Giver of Life and the Comforter, (3) St. Joseph, patron of families and an easy death, (4) St. Christopher, patron of travelers, and finally (5) Our Lady of the Miraculous Medal as seen in a vision given to St. Catherine Laboure ('Mary, conceived without sin, pray for us who have recourse to thee.'). The reverse of the Miraculous Medal is shown on the back of this medal (the cross of Christ above his mother's initial, both above the Sacred Heart Crowned with thorns and the Sorrowful Heart of Mary, depicted (as foretold by Simeon in the Temple) pierced by a sword, (6) Our Lady of Mt Carmel above. The statement regarding calling a priest is to insure every opportunity to receive the last rites should injury occur. At one time priests were readily available within a short distance, but then churches were more plentiful too.
